How to Make an Appointment

Our clinics can see you in-person or via virtual visits. To schedule, call our clinics directly or visit our patient portal, My Health Connection, offered in partnership with UCHealth. Our Make an Appointment page has additional information and scheduling resources.

Canceling or Rescheduling an Appointment

If you need to cancel or reschedule, call your clinic at least 24 hours in advance.

Preparing for Your Visit

Plan to arrive 15-20 minutes early and bring:

  • A valid photo ID
  • Your current insurance card(s) and payment for your co-pay
  • A list of your current medications
  • Any questions or concerns you want to discuss with your provider

Patients arriving more than 10 minutes late may be asked to reschedule their appointment.

Our Plan Your Visit page has more details on what to expect before, during and after your appointment.
