Requesting Records from Another Healthcare Provider
To get your medical records from another healthcare facility, contact them directly. Ask them to send the records to your CU Medicine clinic or provider.
CU Medicine cannot request records for you from outside healthcare facilities or providers.
Requesting Your CU Medicine Medical Records
Adult Patients
Adult patients may request copies of their CU Medicine medical records by:
- Calling their CU Medicine clinic directly
- Using My Health Connection to request records online or through the mobile app
Pediatric Patients
If your child has visited a Children’s Hospital Colorado location, you can request your child’s medical records by:
- Calling your child’s clinic or facility directly
- Using My Chart to request records online or through the mobile app
